Alves Wellness ("we," "us," or "our") is a trade name of The Beauty Bar Skin Care & Makeup Studio, L.L.C., an Arizona limited liability company located at 1200 S Castle Dome Ave, Ste B, Yuma, Arizona 85365. This Privacy Policy explains what information we collect, why we collect it, how we use it, and the choices you have.

1. Information we collect
Information you give us directly
- Name, date of birth, phone number, and email address
- Mailing address
- Emergency contact name and phone number
- Appointment preferences and scheduling information
- Health and wellness information you provide on our intake and screening forms, including medical conditions, medications, implanted devices, pregnancy status, recent surgeries or procedures, and related information
- Your goals for treatment and any notes you choose to share
- Payment information, processed by our payment processor — we do not store full card numbers on our systems
- How you heard about us
- Any messages, emails, or texts you send us

We do not knowingly collect information from anyone under 18 years of age. Our services are for adults 18 and over only.
2. How we use your information
- To schedule, confirm, reschedule, and remind you about appointments

3. Text messaging and your mobile number
We do not sell, rent, share, or otherwise disclose your mobile phone number or your SMS consent to any third party for marketing or promotional purposes. No exceptions.
- We collect mobile phone numbers when you book an appointment, complete an intake form, or otherwise provide your number to us.
- SMS consent is collected through a checkbox that is not pre-selected. You must actively check it. Consent to receive text messages is never a condition of purchasing any service.
- Your mobile number and consent record are shared only with the communications platform and telecommunications carriers strictly necessary to deliver the messages you asked to receive.
- Message frequency varies. Message and data rates may apply.
- To stop receiving text messages, reply STOP to any message. You will receive one confirmation message and then no further texts.

- We retain SMS consent records — including the date, time, and method of consent — for at least four years, as required to demonstrate compliance with federal telecommunications rules.
- Opting out of text messages does not remove you from email communications and does not affect your ability to book appointments.
4. How we share information
We share your information only in these situations:
- Service providers who help us operate — scheduling and customer relationship software, payment processors, email and SMS delivery providers, and website hosting. These providers may use your information only to perform services for us.
- When you ask us to — for example, if you ask us to coordinate with your physician or surgeon.
- When required by law — in response to a subpoena, court order, or other lawful request, or to protect our legal rights.
- In a business transfer — if the business is sold or transferred, client records may transfer as part of that transaction.
We do not sell your personal information. We do not sell or share your health information. We do not sell or share your mobile number.
5. Health information
The health information you provide on our intake and screening forms is used for one purpose: to determine whether our services are safe and appropriate for you, and to deliver them responsibly.
Alves Wellness is a wellness provider, not a healthcare provider, and is not a HIPAA covered entity. We nonetheless treat your health information as confidential and limit access to the personnel who need it to serve you safely.
We do not disclose your health information to anyone outside our business except with your written permission or where required by law.
6. Data retention
- Client records, intake forms, and signed waivers: at least seven years after your last visit
- SMS consent records: at least four years
- Payment transaction records: as required by tax and accounting rules
- Website analytics: typically 26 months
We delete or de-identify information when we no longer have a business or legal reason to keep it.
7. Your choices and rights
- Opt out of texts — reply STOP
- Opt out of marketing emails — use the unsubscribe link

- Request deletion — contact us. We may be required to retain some records (health screening, waivers, financial) even after a deletion request, and we will tell you if that applies.
- You may decline to provide information, but we may not be able to serve you if we cannot complete a health screening.

Our website uses minimal cookies and basic analytics to understand how the site is used. You can disable cookies in your browser settings. Doing so may affect how the site functions.
9. Security
We use reasonable administrative, technical, and physical safeguards to protect your information. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
10. Third-party links
Our website may link to other websites, including our scheduling platform. We are not responsible for the privacy practices of those sites. Review their policies separately.
